By Julia Karow NEW YORK (GenomeWeb) − Genetic tests for hereditary cancer involving one or a few genes are starting to be replaced by multi-gene sequencing panels. But the clinical utility of the new tests, which have been enabled by advances in next-generation sequencing technology, has been unknown. Now, recent studies in various patient groups using panels from Ambry Genetics, Myriad Genetics, InVitae, and an in-house test suggest that multi-gene panels increase the diagnostic yield by about 10 percent over more targeted tests and sometimes deliver unexpected findings in genes not previously associated with a certain cancer type. However, no clinical guidelines exist for many of the genes included in the panels, and the rate of variants of unknown significance is currently high. Breast magnetic resonance imaging (MRI) can help doctors identify cancer lesions and other suspicious areas undetectable on a mammogram or ultrasound of the breasts. Early Surgery in BRCA1 Carriers Lowers Ovarian Cancer Risk Roxanne Nelson February 24, 2014 Women who carry the BRCA1 genetic mutation should undergo prophylactic oophorectomy by the age of 35 to achieve the greatest reduction in their risk for breast or ovarian cancer. This conclusion comes from a large international prospective study published online today in the Journal of Clinical Oncology. But perhaps the most striking finding is the beneficial effect that prophylactic surgery has on all-cause mortality, which has been shown for the first time. By the age of 70, women who underwent the procedure had a significant reduction in their overall risk for death. "To me, waiting to have oophorectomy until after 35 is too much of a chance to take," said lead author Steven Narod, MD, professor of medicine at the University of Toronto, in a statement. "These data are so striking that we believe prophylactic oophorectomy by age 35 should become a universal standard for women with BRCA1 mutations." He noted that ...

In its lead story, NBC Nightly News (2/24, lead story, 2:50, Williams) reported on research suggesting that women who have “a genetic mutation that increases their risk for” ovarian cancer “should make a tough choice about the treatment, even earlier than many do now.” The Wall Street Journal (2/25, Winslow, Subscription Publication) reports that for the study, published online in the Journal of Clinical Oncology, investigators followed approximately 5,800 women who had either a BRCA1 or BRCA2 mutation. In a study reported in the Journal of Clinical Oncology, Kiecolt-Glaser et al found that yoga reduced levels of inflammatory cytokines and fatigue and increased vitality in breast cancer survivors.
At least one in three women diagnosed with breast cancer suffers from lingering fatigue months or even years after treatment ends, but practicing yoga significantly reduces this symptom and also helps lower levels of dangerous inflammation. That’s the finding of a new National Cancer Institute-funded clinical trial that randomly assigned 200 breast cancer survivors to a 90-minute hatha yoga class twice a week for 12 weeks or to a control group waitlisted for the class. At the end of the study, which was published online last Monday in the Journal of Clinical Oncology, those who practiced yoga reported about 50 percent less fatigue than those in the control group and had 20 percent lower levels of three proteins in the blood that are markers for inflammation. A study in the Journal of Clinical Oncology reports there’s been a 225 percent uptick in HPV-positive oropharynx cancers since 1998, according to data from three U.S. cancer centers. The study also predicts that by 2020, oropharyngeal cancer will be the most common HPV-associated cancer in the U.S. surpassing cervical cancer.
